Event: Bankruptcy > Claim Actions OR Creditor Claim Actions > Proof of Claim Attachment - Rule 3001(c)(1)(d) 

Effective 12/1/17, any attachments required by Rule 3001(c)(1) and (d) are filed as a supplement to the claim. Rule 3002(c)(7)(B). To file the attachments, use the event [Bankruptcy > Claim Actions OR Creditor Claim Actions > Proof of Claim Attachment - Rule 3001(c)(1)(d)] .

To File Electronically:

  1. Log into CM/ECF.
  2. Select [Bankruptcy > Claim Actions OR Creditor Claim Actions].
  3. Enter the case number (e.g, xx-xxxxx).
  4. Select [Proof of Claim Attachment - Rule 3001(c)(1)(d) ] from the event list.
  5. Select the party filer. If the party is not listed, click on Add/Create New Party
    Note: The Attorney/Party Association screen may appear. If your party selection was correct, check the box to create an association.
  6. Browse to select the attachment(s) (.pdf file).
  7. Select the related claim from the list.
  8. Enter the name of the claimant.
  9. Modify the text as appropriate
  10. Verify the final docket text; if correct, click [Next] to submit your document(s)
    NOTE: Copies of this notice are immediately e-mailed to all participants who receive electronic notification in the case.
