Event: Bankruptcy > Claim Actions or Creditor Claim Actions > Supplemental Proof of Claim for CARES Forbearance Claim

Official Form 4100S

An eligible creditor can file a supplmental claim in a Chapter 13 Case. The CARES Act includes a provision that allows borrowers experiencing a financial hardhip due to COVID-19 to request a forbearance on their mortgages. This claim can be filed even if the claim's bar date has passed.

This event:

  • Supplements the claim on the cocket and claims register
  • Is not an attachment to the claim
  • Is not an amended claim
  • Is filed as a claims event

Filing Requirements:

  • Form 4100S Supplemental Proof of Claim for CARES Forearance Claim
  • Proof fo Service

To File Electronically:

  1. Log into CM/ECF.
  2. Select [Bankruptcy > Claim Actions OR Creditor Claim Actions].
  3. Enter the case number (e.g, xx-xxxxx).
  4. Select [Supplmental Proof of Claim for CARES Forbearance Claim)] from the event list.
  5. Select the party filer. If the party is not listed, click on Add/Create New Party.
    Note: The Attorney/Party Association screen may appear. If your party selection was correct, check the box to create an association.
  6. Browse to select the Notice (.pdf file).
  7. Select Claim from the list.
  8. Is a Proof of Service included or attached to the document currently being filed? (Yes or No)
  9. Modify the text as appropriate
  10. Verify the final docket text; if correct, click [Next] to submit your document(s).
    Note: The Notice of Electronic Filing displays WITHOUT a document number. Because no document number is associated with this event, the word "doc" appears as a document hyperlink, rather than a document number. This event appears on the Claims Register. Copies of this notice are immediately e-mailed to all participants who receive electronic notification in the case.
