For Appeal Purposes: It is the Appellant's responsibility to submit a Transcript Request Form to the Bankruptcy Court or a certificate stating that the appellant is not ordering a transcript within the time period prescribed in FRBP 8009(a)(1).

To File the Request Electronically:

1. Log into CM/ECF.
2. Select [Bankruptcy > Appeal] or [Adversary > Appeal].
3. Enter the case number (e.g, xx-xxxxx).
4. Select [Request for Transcript] from the event list.
5. Select the party filer.
6. Browse to select the Transcript Request Form (.pdf file).
7. Modify the text as appropriate.
8. Verify the final docket text; if correct, click [Next] to submit your document(s).
   Note: The Notice of Electronic Filing displays giving you the document number. Copies of this notice are immediately e-mailed to all participants who receive electronic notification in the case.
