Each court has an option to display header information on submitted PDF files. (See Figure 1)

The content, color, and location of the header is determined by the court. (See Figure 2)

The displayed PDF is a copy of the original PDF file so the original file is never modified.  The "Include headers" option is disabled if you check the "Create Appendix" option (described below) since headers are always included in an Appendix.
