  1. The first page of the proposed order must have a three (3) inch margin at the top.
  2. There should NOT be a date or signature line for the judge on any page of the order.
  3. At the end of the text of the order, place three pound signs (###) in the center of the page with one blank line separating the last line of text and the centered ###.
  4. Do not include "prepared by" or "drafted by" law firm name at the bottom of the order.
    • Sample proposed order prepared in the correct format (pdf)
    • Sample signed order (pdf)

NOTE: When filing electronically, enter the "Order to be signed on or after" date. Generally, enter day after last day to object or same day order was filed; whichever date is later.

UPDATES:

4/27/2021 Added instruction #4 which states "Do not include "prepared by" or "drafted by" law firm name at the bottom of the order."
