Required if:

• Chapter 7 debtor is an individual
• Chapter 11 debtor is an individual
• Chapter 13 debtor

Note: Chapter 7 individuals with primarily business debts or qualifying military service should also complete Form 122A1-Supp, Statement of Exemption from Presumption of Abuse.

The Statement of Current Monthly Income must be filed with the Petition or within 14 days of filing.

To File Electronically:

1. Log into CM/ECF.
2. Select [Bankruptcy > Miscellaneous].
3. Enter case number (e.g., xx-xxxxx).
4. Select [Form 122 ... Statement of Monthly Income] from the event list.
   
   Note: If an additional Form 122 is required, you may create one pdf and select Ctrl and click to select multiple events. Form 122 events include:
   
   Form 122A-1 Chapter 7 Statement of Monthly Income
   Form 122A-1Supp Statement of Exemption from Presumption of Abuse
   Form 122A-2 Chapter 7 Means Test Calculation
   Form 122B Chapter 11 Statement of Monthly Income
   Form 122C-1 Chapter 13 Statement of Monthly Income
   Form 122C-2 Chapter 13 Calculation of Disposable Income
   
   If amending Form 122A-1 or 122C-1, use:
   
   Amended Form 122A-1 Chapter 7 Statement of Monthly Income
   Amended Form 122C-1 Chapter 13 Statement of Monthly Income
    
5. Select the party filer.
6. Browse to attach the Statement of Monthly Income (pdf file).
7. Answer question re: Presumption of abuse.
   
   If box 2 is checked (The calculation to determine if a presumption of abuse applies will be made under Chapter 7 Means Test Calculation...), then Form 122A–2 must be filed.
   
   If box 3 is checked (The Means Test does not apply now because of qualified military service but it could apply later), then form 122-1Supp must be filed.
8. Modify text as appropriate.
9. Verify the final docket text; if correct, click [Next] to submit your document(s).
