Event: Bankruptcy > Miscellaneous > Employee Income Records (Debtor's Payment Advices)

Local Form: Payment Advices Cover Sheet

All individual debtors must file copies of all payment advices or other evidence of payment received by the debtor from any employer within 60 days before filing the petition. Employee Income Records must be filed with the petition or within 14 days.

Court requests that the Employee Income Records be accompanied by a Payment Advices Cover Sheet. See Policy on Privacy.

Note: When debtor is self-employed or has not received payment from an employer within 60 days before filing the petition, debtor must still file the Payment Advices Cover Sheet.

To File Electronically:

  1. Log into CM/ECF.
  2. Select [Bankruptcy > Miscellaneous].
  3. Enter case number (e.g., xx-xxxxx).
  4. Select [Employee Income Records (Debtor's Payment Advices)] from the event list.
  5. Select the party filer.
  6. Browse to attach the Employee Income Records (pdf file).
  7. Modify text as appropriate.
  8. Verify the final docket text; if correct, click [Next] to submit your document(s).
    Note: The Notice of Electronic Filing displays giving you the document number. Copies of this notice are immediately e-mailed to all participants who receive electronic notification in the case.
