Effective 12/1/17: Local Form 3015-1.1 Chapter 13 Plan and Local Form 3015-1.2 Request to Amend Unconfirmed Chapter 13 Plan

Filing Requirements:

  1. Amended Plan
    THE USE OF LOCAL FORM 3015-1.1 [CHAPTER 13 PLAN] IS MANDATORY FOR ALL CHAPTER 13 PLANS [initial, amended, and modified].
  2. Chapter 13: Request to Amended Unconfirmed Chapter 13 Plan [Local Form 3015-1.2]
    Note: The Request to Amend must be filed with the Amended Plan.
    • Notice of Amended Plan:
    • Chapter 11: Notice of Amended Plan - when required by Rule 3019(a)
    • Chapter 12: Notice of Amended Plan - 21 days when required by 11 USC 1223(c)
    • Chapter 13: Notice of Amended Plan - 21 days to all creditors
  3. Proof of Service
    Note: The Trustee files the proposed order confirming a Chapter 13 plan.

To File Electronically:

  1. Log into CM/ECF.
  2. Select [Bankruptcy > Plan].
  3. Enter the case number (e.g, xx-xxxxx).
  4. Select [Amended Chapter 11 Plan (Prior to confirmation) or Amended Chapter 11 Small Business Plan or Amended Chapter 12 Plan (Prior to confirmation) or Amended Chapter 13 Plan (Prior to confirmation)] from the event list.
  5. Select the party filer.
  6. Browse to select the Amended Plan (.pdf file).
    Note: The Request to Amend Unconfirmed Chapter 13 Plan, Notice, and Proof of Service may be added as attachments to this document.
  7. Refer to all previously filed Plans.
  8. Verify the final docket text; if correct, click [Next] to submit your document(s).
    Note: The Notice of Electronic Filing displays giving you the document number. Copies of this notice are immediately e-mailed to all participants who receive electronic notification in the case.
  9. If notice was not filed with Modified Plan, docket the notice of modified plan along with the proof of service. [Notices > Notice of Amended Plan (before confirmation)]
  10. Chapter 11/12 cases: Docket the Proposed Order Confirming Plan if filed. Instructions
